  I give permission for services and medical treatment at Maine Medical Center Hospital. If I requires services such as emergency room, ambulance, or prescription medication, payment will be the responsibility of the individual stated above.
  
  I fully understand that there are risks involved in the participating in the physical activity related to the participation within the 2025 Maine Footy Soccer Camp. Such participation could put me at risk for getting physically injured (even death) due to the inherent nature of the activity. Knowing this, I fully assume the risks of participation and hereby release and hold harmless from any claim, action, liability or suit arising out of or in any connected with participation against Maine Footy, 2025 Maine Footy Soccer amp, and its Staff and sponsors liable.
